1. A thorough examination of your mouth, teeth and gums
2. A full explanation of your treatment option
3. A written treatment plan (including costs)
4. Advice on how to keep your teeth and gums healthy
5. Information about this practice and the services available

You are responsible for:

1. Attending all appointments
2. Following your dentist’s and dental hygienist’s advice to prevent tooth decay and gum disease
3. Paying your bill on day of treatment, we offer HICAPS for direct claims from health funds
4. Treating our staff with courtesy and respect.

Late Cancellation Policy

Please advise of any changes to your schedule at least 24 hours prior to your appointment.  Any repeat late cancellations or late cancellations without a supporting reason will be chargable.  The fee applied is the treatment value for that appointment.
